If you will be staying in Costa Rica for more than 90 days, then you are required to have a health pass. This document needs to be completed and submitted to the Costa Rican embassy or consulate in your home country before you arrive in Costa Rica. The form consists of two parts – Part A is to be completed by the applicant and Part B is to be completed by a licensed physician.
Here are the instructions on how to fill out Part A of the health pass application form for Costa Rica:
1) On the first page of the form, fill in your personal information including your name, date of birth, gender, passport number and country of citizenship.
2) On the second page of the form, you will need to provide details about your current residence including your address, telephone number and email address.
3) In section 3 of the form, you will need to provide details about your education and employment history.
4) Section 4 of the form requires you to provide information about your medical history including any chronic illnesses or conditions that you have.
5) In section 5 of the form, you will need to list any medications that you are currently taking.
6) Section 6 of the form requires you to list any allergies that you have.
7) In section 7 of the form, you will need to provide details about your travel history including any countries that you have visited in the past 6 months.
8) Finally, in section 8 of the form, you will need to provide two passport-sized photographs and signatures from both yourself and a witness.
