If you wish to file a complaint alleging a violation of the New York State Public Health Law or the Sanitary Code, you may do so by writing to the Department of Health. Your complaint should be addressed to the attention of the Bureau of Regulatory Affairs. Please be sure to include as much specific information as possible, including the name, address, and phone number of the person or facility involved, as well as a description of the problem. You may also include any other relevant information that would assist us in our investigation.
What is the Department of Health?
The Department of Health is a state government agency responsible for protecting the public’s health. The department investigates complaints about health care facilities and providers, and takes action to improve the quality of care. The department also licenses and regulates health care facilities and providers, and sets minimum standards for their operation.
What is the complaint process?
The first step in filing a complaint is to call the Department of Health at 1-800-566-1335 and speak with a customer service representative.
The customer service representative will ask for your name, address, and phone number; the name, address, and phone number of the business or individual you are filing the complaint against; a description of the problem; and the names of any witnesses. The customer service representative may also ask for your email address so that we can send you updates on the status of your complaint.

After you have filed a complaint, a Department of Health investigator will contact you to discuss your complaint and gather additional information. The investigator will also contact the business or individual named in the complaint to get their side of the story. After conducting an investigation, the investigator will determine whether there are any health code violations and, if so, work with the business or individual to fix them.
